Thin Melanoma (c18t3)

c18t3P1 (case 2365): The epidermis shows lentiginous and junctional patterns. Atypia is moderate in the lentiginous and junctional component; the cells are pigmented. The papillary dermis is only slightly widened; it shows only mild inflammatory infiltrates. The epidemis is slightly hyperplastic with elongation of rete ridges. A portion of a polypoid component is represented near the right margin of the field.

c18t3P2: From the character of the patterns in this field, I would guess that this is a lesion of the eyelid and that the conjunctiva is involved by the melanocytic dysplasia. There are band-like infiltrates of lymphoid cells.

c18t3P3: The polypoid portion of the lesion shows broad nest of rather uniform cells in a widened papillary dermis. The patterns are “nevoid.” There are nevus-like fissures in some of the nests. Nests extend into the reticular dermis (pattern IV), but there appears to be maturation at this magnification. Lymphoid infiltrates are minimal.

c18t3P4: The papillary dermis is widened and delicate. The nests of melanocytic cells are broad and interconnected. Some of the nests are fissured. There are mild lentiginous and junctional components in the epidermis. The lesion is nevoid in a common nevus-like pattern.

c18t3P5: At high magnification, the cells are polygonal. They have pale acidophilic cytoplasm and atypical, eccentric nuclei. The cells are loosely attached to their neighbors. Flattened cell outline many of the nests. The nuclei of the neoplastic cells are enlarged, irregular in outline, and hyperchromatic. The lesion qualifies as a common nevus-like nevoid melanoma (MDM). It measured 0.83 mm in vertical dimensions. It was associated with a nodal metastasis.
