Thin Melanoma (c17t3)

c17t3P1 (case 1593 cont.): This is a continuation of the presentation of the second case in c16t3. There are prominent lentiginous and junctional patterns at the dermal-epidermal interface. Rete ridges are elongated. The neoplastic cells form nests and fascicles at the dermal-epidermal interface. The cells are pigmented. Lymphoid infiltrates at the advancing margin are moderate. The basic pattern at this magnification (pattern analysis) is nevoid.

c17t3P2: Individual neoplastic cells and nests of neoplastic cells have migrated upward into the epiderms. Some of the neoplastic cells extend into the upper portion of the reticular dermis. Although the degree of atypia is marked, the nesting patterns provides a “nevoid” quality. The lesion might be characterized as a common nevus-like MDM variant. Focally, some of the neoplastic cells to the right of the field have extended into the upper portion of the reticular dermis (level IV pattern); there is some degree of maturation in this area.

c17t3P3: In the epidermis, the lesion shows patterns of the common final pathway; cytologic atypia is marked. In the dermis, there are patterns of maturation. The stroma is delicate. Nests of cells focally are regularly spaced in the pattern of an early vertical growth component. Lymphoid infiltrates are mild. In the dermis, the patterns have a nevoid quality; the lesion qualifies as a MDM of nevoid type.
